VITA - Information and Documents required
Clients should bring the following when they come for their appoinment.
- Names and social security numbers (SSN) and birth dates for each adult and child (the names and SSN must agree with the information contained in the Social Security Administration’s data files). If you were married or divorced during the year and changed your name, you must make the change with Social Security.
- Wage and earning statement(s) Form W-2, W-2G, 1099-R, from all employers;
- Forms 1099, for such things as interest or dividends;
- Records of other income (example: jury duty fees, gambling winnings)
- Records of any alimony received or paid (must provide name and SSN of ex-spouse)
- Current year’s tax package if you received one;
- A copy of last year's tax return (both federal and state if available);
- Notice from the IRS of your 2003 advanced payment for the child tax credit ($400 per qualified child)
- If you want to claim deduction for day care expense - Name, address, and federal identification number or SSN of business or person providing day care services;
- If you want direct deposit of your refund bring the account and routing numbers of the bank account to which you want the deposit to be made;
- Any other information pertinent to this year's tax return
